- Pregnant woman, breastfeeding mother, or mother of a child under 2 years.
- Resident of one of the selected 13 districts of Punjab.
- Belongs to a low-income household (registered in NSER/BISP).
- Must have a valid CNIC.
- Willing to attend regular maternal and child health checkups at government health facilities.
- Registration must be done at a BHU, RHC, THQ, or DHQ hospital.
